Community outings

Getting out of the house and into the world — with someone who makes it easy rather than stressful.

Who you'll be dealing with

One familiar face, not a roster

When you contact LCT, you get me — Tony. Not a call centre, and not a different worker every fortnight.

Over two years of one-to-one disability support and youth mentoring across Adelaide, with a background in education and training behind it. If something isn't working, you'll hear it from me early rather than late.

I speak Vietnamese and English, so families who'd rather talk things through in Vietnamese can do that directly with me — no interpreter, no waiting on someone else to translate.

How it works

Getting started is simpler than you think

From first contact to first session — here's exactly what happens.

  1. Get in touch

    Call, email or send the referral form. Self-referrals, families, support coordinators and other providers are all welcome.

  2. Free intro chat

    We talk about the person, their goals, their plan and what good support looks like for them. No jargon, no pressure.

  3. Meet & match

    We propose a worker or mentor we think fits, and arrange a meet-and-greet. If it doesn't click, we try again.

  4. Set the plan

    Goals, hours, funding and a simple service agreement — written in plain English so everyone's on the same page.

  5. Start support

    Sessions begin. We keep notes, stay in contact and adjust as things change.

  6. Review & grow

    We check in regularly on what's working, and report progress in a way that's useful at plan review time.
